If the evaluation is ordered by the court or a probation officer, clients are required to bring a copy of the Police Report from the incident and a recent BCI Report. Completed Evaluations will be sent to the referring agency within five business days.

- You must obtain your police report prior to evaluation. Contact the court associated with the city in which the offense occurred and request it via email.
